• E’ una malattia molto rara che è stata riscontrata solo in pazienti con insufficienza renale grave (GFR <30 mL/min/1.73 m2) e insufficienza renale acuta
• Colpisce prevalentemente la CUTE con:– chiazze rosse o brune o papule– ispessimento della pelle degli arti e del tronco – difficoltà di movimento, contratture e immobilità– gonfiore alle mani e piedi; possono comparire lesioni vescicolari – bruciore, prurito, dolore acuto nelle zone colpite
• Può colpire anche organi non cutanei come POLMONI, FEGATO, MUSCOLI E CUORE

L’ etiologia• L’etiologia della NSF non è nota con chiarezza, ma sembra essere MULTIFATTORIALE• Sono stati considerati elementi scatenanti:
- Chirurgia e/o comparsa di trombosi o di altro danno vascolare (Cowper 2003);
- Stati infiammatori o pro-infiammatori (Sadowski 2007);
- Somministrazione di elevati dosi di eritropietina (Swaminathan 2006)– EPO has fibrogenic properties;– Patients who developed NSF received more than 2x the normal EPO dose;– EPO use increased after the 1997 Dialysis Outcomes Quality Initiative guidelines;
- Più recentemente l’uso di mdc a base di gadolinio (Grobner 2006), sebbene esistano report che documentano episodi di NSF senza esposizione preventiva al Gd (Wahba 2007, Collidge 2007, Todd 2007).
• 9 pz sottoposti ad angio-RM in 2 anni• 5/9 sviluppano NFS in 2- 4 settimane• MdC: gadodiamide• 5 pazienti presentano bicarbonati < 21 (media
19.7)• Ipotesi: acidosi + GD

• Gadolinium is released from the chelate
• The higher the amount of free gadolinium released in the body – the lower the stability, – the higher the dose, – the slower the elimination of the chelate from the
body
• The gadolinium released from the chelate may stay in the skin of patients for as long as 11 months following the admnistration of a gadolinium chelate

• a likely cause of NSF is transmetallation, which involves the breaking of the bond between the toxic metal gadolinium and the ligand (or chelate), the part of the contrast agent that is supposed to block the escape of the free gadolinium ion
• the available studies indicate that agents with the cyclic structure have the strongest bonds between the gadolinium ion and the ligand molecule. Therefore, the cyclic agents may be safer than linear agents with respect to NSF.E.Kannal, ARRS Society Meeting, Blue Ribbon Panel on MR

Epidemiologia
• In pazienti con eGFR < 15 l’emodialisi è associata ad una significativa riduzione di NSF
• Nessun paziente sottoposto a emodialisi nel medesimo giorno dell’esposizione al Gd ha sviluppato NSF
• L’incidenza di NSF in pazienti che hanno ricevuto una dose standard (0.1 mmol/Kg) è 0 su 74,124 pazienti (86% Gadodiamide)
• L’incidenza è stata inferiore in pazienti trattati con dose singola ripetuta (0 in 5725) piuttosto che in quelli sottoposti a singola dose elevata (9 in 5119)
Epidemiologia
•Non esiste una terapia mirata•Il miglioramento della funzionalità renale riduce o blocca la progressione di malattia
•L’emodialisi sembra in grado di rimuovere il Gd- sino al 98% in tre sessioni
•Trapianto renale

Il problema della dose
• In Japan, high-dose (>0.2 mmol/kg weight) administration of GBCAs is extremely uncommon.
• In addition, contrast-enhanced MR angiography, which usually needs a relatively large amount of GBCAs, is less commonly performed in Japan.
• However, underdiagnosis of NSF is unlikely to explain the numerical discrepancy of NSF incidence.
• The Japanese experience supports the theory that the high-dose administration of GBCAs may be a risk factor for developing NSF , and the recommendation of the European Society of Urogenital Radiology is appropriate:
• "In patients with CKD [chronic kidney disease stage] 4 and 5 (<30 mL/min), always use the smallest possible amount of the contrast agent to archive an adequate diagnostic examination, and never use more than 0.3 mmol/kg of Gd-CM”

- è fortemente suggerito in tutti i pazienti con note anamnestiche sospette per insufficienza renale
- è assolutamente indispensabile in tutti i pazienti con storia clinica di IR, per definire il grading della malattia
Il valore della creatininemia non deve essere antecedente ai 30 gg, a meno di significative modificazioni del quadro clinico

• Based on current knowledge it is estimated that patients with severe CKD have a 1% to 7% chance of developing NSF after exposure to GBCM (gadodiamide) ……….
• There is no proof that any GBCM is completely safe in this patient group (CKD 4 or 5); however, at the present time, should GBCM use be required in these patients, some have suggested avoiding gadodiamide and considering use of macrocyclic agents.
• Currently, there is no evidence that patients in these groups (CKD 1 or 2) are at increased risk of developing NSF. Current consensus is that all GBCM can be administered safely to these patients.

WARNINGS per Gd ad ALTO RISCHIO:-Non utilizzare nei pazienti con gravi problemi renali e nei neonati di meno 4 settimane-Dosaggio limitato alla dose minima raccomandata nei pazienti con danno renale moderato e nei bambini fino ad 1 anno di età con intervallo di almeno 7 giorni-Sospendere l’allattamento per almeno 24 ore dopo il contrasto-Esami di laboratorio prima del contrasto

WARNINGS per Gd a BASSO e MEDIO RISCHIO:- Aggiungere avvertenze alle informazioni sulla prescrizione per l’uso in
pazienti con gravi problemi renali e nei pazienti sottoposti a trapianto- Dosaggio limitato alla dose minima raccomandata nei pazienti con danno
renale grave, nei pazienti sottoposti od in attesa di trapianto e nei bambini sino ad 1 anno si età con intervallo di almeno 7 giorni
- Medico e madre devono decidere se continuare o sospendere l’allattamento per almeno 24 ore dopo l’esame
- Esami di laboratorio prima del contrasto

1. avoiding high doses of GBCA (>0.1 mmmol/kg)
2. avoiding non ionic linear chelates in pts undergoing dialysis and in pts with GFR <30 ml/min, especially in the setting of pro-inflammatory conditions
3. dialyzing quickly after GBCA administration for pts already in dialysis
4. avoiding GBCA in acute renal failure, especially while serum creatinine level is rising
The reduction in risk may be attained with each of the following:
In stage 5 CKD pts pre-dialysis we often delay the procedure until the patient starts dialysis. Both iodinated contrast and GBCA may be used, but GBCA should be riskless.
Hemodialysis possibly reduces risk in pts who need a GBCA in spite of the risk of NSF

I believe that the risk of NFS is minimal at best by utilizing the following guidelines:
1. avoid gadodiamide, gadoversetamide and gadopentetate (FDA 2010)
2. do not exceed 0.1 mmol/kg dose
3. hemodyalize the day of and two consecutive days following Gd exposure
and I believe, “YES”, with reasonable certainty gadolinium can be given safely to pts on dialysis

Nei pazienti ad alto rischio (GFR<30 ml/min, dializzati e trapiantati o in lista per trapianto epatico):
1. giustificare l’adozione della procedura
2. impiegare i mdc a medio e basso rischio (EMEA 2009)
3. dose minima possibile
4. evitare l’utilizzo off label del Gd
5. rispettare un idoneo intervallo temporale fra due somministrazioni

• La NFS è una malattia rara
- 200 milioni di somministrazioni
- 600 casi riportati in letteratura (425 casi ICNFDR)
- quanti casi sub-clinici?
• Si sviluppa esclusivamente in pazienti con compromissione della funzionalità renale
• Esiste una significativa differenza nella prevalenza di NFS con i diversi MdC
– “Does low risk equal no risk?”
Cosa sappiamo ad oggi
• La dose cumulativa di Gadolinio, sia per quantità somministrata in singolo esame sia per ripetute dosi in un tempo relativamente breve, sembra avere un ruolo cruciale nel determinismo di NSF
– Ma esistono casi associati alla singola esposizione con singola dose
• Molti pazienti “ad alto rischio” non sviluppano NSF
• Il numero dei nuovi casi è drasticamente ridotto dopo il 2007
