nelle metanalisi dei principali studi registrativi (2,3)

Conclusioni
Le considerazioni finali che ci sentiamo di poter fare sono:
Durante la terapia con Teriparatide, ( primi 18 mesi) aumenta
significativamente la BMD, con un incremento maggiore a carico della
colonna rispetto al femore.
Durante i 24 mesi si sono avuti solo 5/141 nuove fratture (3V e 2F) Con una
protezione del 96,5%.
Il consumo dei FANS si è ridotto in modo significativo sin dal sesto mese di
trattamento (-58%)
La qualità della vita, valutata col Qualeffo è migliorata in modo
considerevole.
Sei mesi dopo la sospensione del Teriparatide, si è registrato un ulteriore
lieve incremento della BMD a livello della colonna (+1%) mentre si è
avuto ancora un ulteriore incremento a carico del femore (+3,8%).
Sei mesi dopo la sospensione del Teriparatide il consumo dei FANS è
rimasto pressocchè immutato , mentre la qualità della vita
è leggermente peggiorata.
Which therapy after 18 months of treatment with teriparatide in severe
senile osteoporosis?

Severe osteoporosis is a form of osteoporosis that seems not to be responding to antiresorptives
therapy. In fact, we have 10-25% of old aged patients defined “non responders” as they don’t have any
increase of BMD and moreover they continue to have new fracture at both vertebral and non vertebral site.
In a previous publication by our working group, we observed that teriparatide administration for 18
months in postmenopausal osteoporotic patients was efficient in either increase new bone mass (+10%) or
reduce new fractures risk (-96%). Teriparatide was efficient also in improving quality of life (+50%).
At the end of these 18 months, we followed up patients every 6 months, monitoring BMD, fracture risk
and quality of life. About 30% of patients (n=45) remained on Calcium + Vitamin D and followed up. After 1
year over the 18 months, we observed a gradual deterioration of quality of life while BMD reached a plateau
with a trend towards a decrease of bone mineral density at both sites, vertebral and hip. So we decided to
divide these 45 patients (all females , mean age 78±6) into 3 groups (A,B and C) of the same
numerousness. We started a therapy with alendronate ( 70 mg/weekly) for group A, with Risedronate (35
mg/weekly) for group B and Strontium Ranelate (2g/die) for group C. We analyzed the effect of therapies
after 6 months and after 1 year performing 1) a lumbar BMD 2) a hip BMD 3) administration of QUALEFFO
with the 41 items questionnaire to evaluate the quality of life.

CONSIDERAZIONI :
1) 141 pazienti con OSS, trattati 18 mesi con TERIPARATIDE, hanno avuto
solo 5 fratture durante il trattamento (3V e 2F) protezione = 96,5%
2) 96 per altri 24 mesi hanno assunto e continuano ad assumere solo Ca+D
e non hanno avuto fratture (protezione 100%)
3) 45 pazienti trattati per 12 mesi hanno assunto solo Ca+D e per 12 mesi o
Al o Ris o Sr hanno avuto 3 fratture V (protezione 93%)
CONCLUSIONE FINALE :
In 141 pazienti, trattati tutti 18 mesi con PTH, seguiti per altri 24 mesi, 96 dei quali
hanno assunto solo Ca+D e 45 anche o Al o Ris o Sr, nei 42 mesi complessivi
di trattamento si sono verificati soltanto 8 fratture (6V e 2F), come a dire si è
avuta una protezione complessiva sulle nuove fratture del 94,33%
In termini di farmaco economia una spesa di 8,46 euro/die ( PTH +Ca+D+Al+Ris+Sr)
per 42 mesi nei 141 pazienti affetti da OSS , ha ridotto di oltre il 94%,
l’insorgenza di nuove fratture, mentre ad oggi la protezione media con tutte le
terapie usate ( da 1 a 2 euro/die) a 3 anni oscilla intorno al 40-50%.
