Un problema di sostenibilità
Il costo delle terapie antitumorali è in rapido e costante aumento, con un’incidenza tra le più alte sull’intera spesa farmaceutica.
In Italia, il costo medio di un trattamento è passato dai 3850 € circa della seconda metà degli anni Novanta ai 44.900 € della prima metà dello scorso decennio e le proiezioni per il 2020 stimano un costo compreso tra i 70.000 e i 100.000 € per Paziente.

• Agisco sempre nell’interesse del Paziente! Questo è il mio compito. Voglio proprio vedere chi potrà muovermi addebiti!
• Prescrivo sempre il trattamento migliore, indipendentemente dai percorsi autorizzativi (che posso provare ad eludere...)

The Scagliotti’s study found similar OS between the groups: 10.3months, establishing non-inferiority (hazard ratio [HR], 0.94;95% CI,0.84-1.05; upper margin for noninferiority, 1.18).
However, the authors analyzed survival differences in 19 subgroups, including histology, and found that for adenocarcinoma OS was significantly increased with cisplatin-pemetrexed combination vs cisplatin-gemcitabineby 1.7 months(12.6 vs 10.9months;HR,0.84;P = .03). Forsquamous lung cancer, the trend was opposite (9.4 v 10.8 months;HR, 1.23; P = .05).

• While pemetrexed improved OS in the nonsquamous group (but not squamous group) it did so without an improvement in progression-free survival (PFS).
• there is no reason for a drug like pemetrexed (unlike immunotherapy) to increase OS without increasing PFS. Empirical data confirm this event is unlikely.
• The study did not adjust for multiple hypothesis testing, and the authors conducted at least 19 subgroup analyses.
• subgroup analyses should always be considered hypothesis generating and not definitive.
• Furthermore, when multiple subgroup analyses are performed and 1 or 2 comparisons produce significant results, it is highly likely that those positive findings are due to chance alone.

Drug prescription appropriateness in the elderly
• Drug prescription inappropriateness increases the prevalence of drug-induced harms, with consequences related to patients’ health and cost of care.
• Polypharmacy and inappropriate medication prescriptions are associated with increased morbidity and mortality.

Prescrizione dei farmaci biosimilari
• La legge definisce i confini del dato tecnico.
• La prescrizione dei farmaci, specie di quelli ad alto costo, deve essere proporzionata, ragionevole e trasparente.
• Qualora il medico scelga il farmaco originatore invece del biosimilare, quale giustificazione può addurre per spiegare la maggiore spesa, visto che EMA prima ed AIFA dopo hanno garantito la equiefficacia e la equitossicità?
