Il cateterismo vescicale è una metodica di drenaggio attaall’evacuazione dell’urina in tutte quelle situazioni in cui nonsia possibile farlo fisiologicamente.
L’esposizione al cateterismo vescicale è associata ad un consistente aumento del rischio di Infezioni delle Vie Urinarie e di complicanze non infettive
Le IVU associate a CV a lungo termine rappresentano la piùcomune causa di infezioni acquisite in ospedale (Warren1994)

• The first step in reducing catheter-associated urinary tract infections and other complications is toavoid unnecessary catheterisation; the second is to remove the catheter as soon aspossible. (Pratt 2007)
• There are two accepted basic principles: keeping the catheter system closed; and removingthe catheter when it is no longer needed.
• Systemic antibiotics are also effective in the long-term catheterised patient, we need to find outwhether the protective effect of prophylactic antibiotics (used continuously) counterbalances theside effects, particularly the emergence of bacterial resistance to multiple antibiotics. Possiblestrategies to reduce these disadvantages are limiting antibiotic use: a) to patients with apositive urine culture (usually set at a predefined density of bacteria), b) to those withclinical symptoms (such as pain or fever), or c) to certain circumstances such as whenchanging the catheter. Patients who are at high risk for complications of urinary tractinfection should particularly be included.

RIMOZIONE PRECOCE
L’incidenza di batteriuria catetere associata oscilla dal 3 al 10%per ogni giorno in cui il catetere vescicale rimane in situ, pertantola maggior parte dei pz è a rischio di sviluppare una batteriuriaentro 30 giorni .
Il CV deve essere rimosso quando non è più necessario,attraverso un riesame periodico delle condizioni che lohanno reso indispensabile.

Thre was not enough evidence to suggest wether or not any standard catheter was better thananother in terms of reducing the risk of bacteriuriain hospitalized adults catheterized short term.Silicone catheter may be less likely to cause urethral side effects in men:howevwer, this resultshould be interpreted with some caution as the trials were small and the outcome definitions andspecific catheters compared varied.The results suggest that the use of silver alloy indwelling catheters for catheterising hospitalisedadults short-term reduces the risk of catheter associated bacteriuria……..Therefore this evidencemust be treated cautiously. There was insufficient data regarding the effect of silver alloycatheters on incident of catheter –associated UTIWhile it is widely accepted that systemic antibiotics reduce the risk of catheter-associated urinarytract infection in short-term catheterised patients (Huth 1992; Pfefferkorn 2009) it is uncertainwhether this protective effect offsets the risk of developing side effects from the antibiotics and theemergence of bacterial resistance.
