Farmacocinetica dell’ENF
Low doses of heparin clear rapidly from plasma through a saturable (cellular) mechanism and t he slower, nonsaturable, dose-independent mechanism of renal clearance. Very high doses of heparin are cleared predominantly through the slower nonsaturable mechanism of clearance

Options available for reversal oforal anticoagulation
• Fresh frozen plasma (FFP)
Contains factors required Available in most centers Risk of fluid overload and
viral transmission
• Prothrombin Complex Concentrates(e.g., Beriplex P/N, Prothromplex )
Contain all factors required Fast application possible Low volume Predictable and measurable effect May trigger thrombotic events viral transmission
Time is required to thaw and transfuse the product
Duration of infusion requires patient monitoring
May fail to completely reverse anticoagulation
May trigger thrombotic events Moderately expensive
• rFVIIa (Novoseven )
Low volume Fast application possible Dose not established Short lasting effect May trigger thrombotic events Very expensive
ACCP Evidence-based Practice GuidelineAnsell et al. Chest 2008
• In patients with life-threatening bleeding
“…. we recommend holding warfarin therapy and administering fresh frozen plasma, PCC, and administering fresh frozen plasma, PCC, or recombinant factor VIIa supplemented with vitamin K, 10 mg by slow IV infusion, repeated, if necessary, depending on the INR” (Grade 1C)

FCSA, 2006
La ripresa della TAO
Rivalutare l’indicazione al trattamento
Rivalutare l’INR target

Oral, once-daily direct Factor Xa inhibitor with predictable PK
• Fast onset of action – no substantial accumulation after multiple doses• Half-life 5–9 hours in healthy young subjects, 11–13 hours in elderly subjects• Low intra-individual PK variability/moderate inter-individual PK variability• Rivaroxaban has a dual mode of elimination:
– 2/3 metabolized by the liver – 1/3 excreted unchanged by the kidneys– No major or active circulating metabolites

Managing bleeding
• No antidotes
• Half -life dependent on renal function• Half -life dependent on renal function
• Utility of plasma or procoagulants uncertain
Mean half life of current drugs
• LMWH: 3-6 hours• Fondaparinux: 13-21 hours• Warfarin: 36 -42 hours• Warfarin: 36 -42 hours• Rivaroxaban: 7-11 hours• Dabigatran: 12-14 hours
Recommended strategies
• Rivaroxaban– Blood product or component transfusion– rFVII may be considered– rFVII may be considered
• Dabigatran– Blood product or component transfusion– rFVII may be considered
Potential management strategies
• Non-specific prohemostatic agents– Antifibrinolytics (e.g. tranexamic acid)– DDAVP– rFVIIa– Activated prothrombin complex concentrates– Activated prothrombin complex concentrates
• Removal of the anticoagulant– Hemodialysis*– Hemoperfusion– Plasmapheresis
Crowther and Warkentin JTH 2009*Dabigatran can be dialysed, Rivaroxaban likely not
