presentato all’EHA?

Quali biomarkers
• 17p- mediante FISH su PB
• Mutazioni TP53 se FISH neg per 17p-
• Mutazioni IGHV (non cambia, è riproducibile), non fa scegliere terapia
• Beta 2 microglobulina e stadio clinico
In quali pazienti
terapeutiche
Organizzazione della diagnostica citogenetico-molecolare su base
regionale/macroregionale

MRD-negativity may indicate deeper remission
Baseline tumor burden
SD

PR Decrease 50–<100% = PR
CR

Detection limit of standard

Definition of MRD-negativity
(< 1 CLL cell in 10,000)
MRD+ CR
MRD– CR
Deeper remission
Hallek M, et al. Blood 2008; 111:5446–5456.
≥ 50% increase = PD
* May be PR for cytopenia or organ enlargement
MRD-negative patients have fewer CLL cells after treatment

Increased efficacy translates into improved survival
Long term remissions with FCR
Are Ig-mutated CLL patients being cured by FCR?
• High proportion of Ig-mutated patients free of clinical relapse at 11 years
• By late 2013 70 Ig-mutated patients available to study residual disease status at 11 yrs still have no molecular evidence of disease

LANDMARK ANALYSIS OF PFS AT FINAL MRD ASSESSMENT
1. MRD3-: ≤3FCR and MRD neg
2. MRD3+: ≤3FCR and MRD pos
3. MRD3-/end-: >3FCR and MRD- at month 3 and end
4. MRD3+/end-: >3FCR and MRD+ at month 3 and MRD- at end
5. MRD3+/end+: >3FCR and MRD pos at 3 and at end
Patients with MRD-negative status had a longer PFS than patients with MRD-
positive status receiving 3 (P<.001) or more (P=.05) courses of FCR.
